Hospitals are meant to provide acute care needs for those experiencing medical instability. Sometimes a patient may become medically stable for discharge from the hospital but not be functionally stable, and their discharge from the hospital becomes delayed. We want to help hospitals feel comfortable discharging patients back to the community if they need close follow-up or additional services to be successful post-discharge.

Our case managers will come to the hospital bedside and collaborate with any inpatient care managers to ensure a fluid transition out of the hospital.

With talented healthcare workers  our community, we want to ensure that the interventions and recommendations that happen in the clinic office can be maintained with confidence when your patient leaves.

We encourage clinical providers, medical assistants, PCPs, etc...to reach out to us if there is a patient who could use additional management of their healthcare needs to obtain positive outcomes in the short- and long-term.
