We are here to take the stress off your plate by finding, coordinating, and advocating for the right care.​
Our case managers are Licensed Clinical Social Workers, Registered Nurses, and have accreditations from the American Case Management Association.
Care Coordination
Finding the best care for yourself or a loved one requires attention to details, organization, and a knowledge of the healthcare community. We bring that attention to your individual case. Services Include: Establishing care with medical providers, including primary care and specialty care ​Managing medical appointments and facilitate communication amongst various providers to ensure patient-centered care, mitigating duplicated or contraindicated services Conducting a comprehensive evaluation of acute and chronic medical and psychiatric disorders, health literacy, ADL status, family support Fulfill referrals for specialty services (such as physical therapy, dialysis, behavioral health) and aide in acquisition of durable medical equipment and supplies Develop a care plan to achieve short- and long-term health goals related to nutrition, medication therapies, mental health​

Services include: Assistance with enrolling in state and federal health care plans Facilitating collection of verification documents as needed for different application processes Provide assistance with medical bill management, payment plans, and understanding coverage options Assistance with renewal of benefits, applying for community-based programs
Advanced Care Planning
Advanced Care Planning refers to how you would like important matters in your life to be handle in the future. This includes decisions related to your medical treatment as well as financial decisions. We review different types of advanced directives with you and your loved ones so that your wishes for yourself and your family are protected. Discuss and assist with documentation of wishes surrounding scope of medical treatment Complete petition for appointment of a guardian or conservator for individuals Provide education on and create advanced directives including: Medical / Durable Power-of-Attorney, Medical Orders for Scope of Treatment, CPR Directive​s
