
Zeina Toure, Owner
My name is Zeina. I am a social worker, and I am by your side. Much of my experience as a social worker was spent in a local hospital in Denver working with patients in the intensive care units. I met people who were sometimes experiencing some of the hardest days of their lives. One thing I always found hard was that when a patient left the ICU, my work with them as a social worker would end. Families would ask me if I would still be their social worker when they went to the next unit. "No." I would say, "But you'll be in good hands." The golden reassurance in healthcare.
The family would joke "Oh, darn." A partial joke. I remember what it was like when my family member was hospitalized and we found the one person who finally heard our concerns and our need for support. When you find trust in a healthcare industry, you want to hold on. It can take so long to explain your situation again, to try to present your reality in a way that the new face in front of you would have to care -- even though you can't imagine that someone wouldn't.
I wondered then, why did we fragment this support? Why not have someone who could remain alongside families to help navigate care and access important health resources, no matter where in the journey they might be?
I created New Phase Navigation to do exactly that.
To those experiencing a health care transition, whether yours or loved one's, remember: You don't need to convince me that what you're going through is important. I know that it is, and it deserves care and attention. That's why I stay by your side.

Zeina is the founder and owner of New Phase Navigation. She created the vision of a community care team from her dedication to community work and from seeing some of the connections that were formed in healthcare settings that she felt would be valuable to have value in the community. She has over 10 years of experience as a clinical social worker at public safety net hospital, supporting families in answering questions about next steps in light of their health conditions. Zeina enjoys a little bit of everything. She loves to learn about different cultures and to try new foods. She plays the saxophone and likes seeing live music performances.

Kymberly Ward
Case Manager
Kymberly is one of our trusted case managers. She provides you with a reassurance and perspective as you lay out a plan of care. Kym's calming presence and critical thinking have put families and workers alike at ease. She will take her time listening to your concerns and help you create a plan for what is important to you in a timeline that is important to you.

Rachael Sheaffer
Case Manager
Rachael is our Lead Case Manager. She has over a decade of experience as a social worker and a supervisor as a local hospital and spends time getting to know her community hands-on and finding resources that can be useful for her clients. Rest assured that if Rachael has a resource suggestion for your, she has taken the time understand it thoroughly. If you don't believe me, ask her two cats!

Rachel Seiger, LSW
Rachel journeys alongside her clients with presence, curiosity, and deep listening. Her approach is grounded in empathy, the wisdom of the body, and a belief that each person is the expert of their own life.
With a background rooted in medical and hospice social work, Rachel has supported individuals and families through a wide range of health transitions, grief, and end-of-life experiences. She has worked closely with aging adults, care teams, and loved ones to provide emotional support and connect people with meaningful resources.
Rachel’s therapeutic style is flexible and holistic, drawing from psychodynamic therapy, somatic experiencing, parts work, mindfulness, attachment theory, emotionally focused therapy, and trauma-informed care. She recognizes that our environments—past and present—shape how we move through the world and how the body carries our stories.
She works with clients navigating caregiver burnout, grief, adjustment to illness, anxiety, depression, and relationship challenges. Above all, Rachel aims to create a space where clients feel safe, supported, and seen—a place where healing can unfold at its own pace.
