Frequently Asked Questions
Are you a home care agency?
Nope—we’re not a home care agency. We don’t provide medical or hands-on personal care like bathing or administering medications. Instead, we focus on case management and non-medical support. Think of us as your behind-the-scenes team: coordinating care, solving logistical problems, and providing emotional and practical support to make life a little smoother.
Do you help people outside of the Denver area?
Yes! While we’re based in Colorado, we work with many families who live in other states but have loved ones here. We often serve as the local support system—checking in, reporting back, and being there when you can’t be.
Can you help with guardianship or conservatorship?
Yes. We can file the petition paperwork with the appropriate court system, write letters of necessity, and testify in court to support a need for a guardian or conservator.
What are your rates?
We charge hourly rates based on the service type and level of involvement. You can find full pricing on our Rates Page, or we’re happy to discuss your needs and create a plan that works for you.
Do you work with younger adults, or just older people?
We support adults of all ages. While many of our clients are older, we’ve also worked with folks in their 20s and 30s who need help navigating a tough time—like a health crisis, disability, or recovery from a mental health stay.
Can you support someone with mental health needs?
Yes. Our Support Companions can be a consistent and trusted person for check-ins, structure, and social support. We also help with community integration after a hospital stay or big transition, and our case managers can coordinate mental health services if needed.
What makes New Phase Navigation different?
We combine logistical support and emotional presence in a way that most agencies don’t. We’re here to help real people with real lives—not just check boxes. Our team steps in with compassion, clarity, and follow-through when it matters most. You won’t get lost in a system here.
What services do you provide?
We offer two main services:
-
Case management, which helps with things like navigating healthcare systems, figuring out next steps, handling paperwork, or finding the right resources.
-
Support companions, who provide consistent emotional support and help with everyday things like rides, companionship, home organization, and check-ins.
Together, they create a team that can step in during transitions, support people day-to-day, and give caregivers peace of mind.
How do you stay in touch with families?
We use a mix of phone calls, scheduled updates, and a secure online portal to keep everyone in the loop. This is especially helpful for out-of-state family members who want to stay connected and involved without needing to manage every detail themselves.
What if it doesn’t work out with NPN?
We get it—inviting someone into your life is a big deal. That’s why we offer a risk-free trial period: if we’re not addressing your needs in the first month, we’ll offer a refund for unused hours or work with you to make it right. We want this to feel like a win-win, not a gamble.
How do case managers and support companions work together?
Your case manager is your go-to person for big-picture planning—whether that's finding services, solving problems, or navigating paperwork. Your support companion is the steady presence who shows up, helps with day-to-day tasks, and checks in regularly. Together, they form a supportive circle around you (or your loved one).
Do you accept insurance?
We don’t accept insurance, but we offer different pricing levels and always try to work with you to find something that fits your budget. You can visit our Rates page for more details or reach out to talk through options.
How Can I Get Involved?
Yes! If you’re someone who’s compassionate, flexible, and passionate about helping others, we’d love to hear from you. Visit our Careers page to see current openings.
